We understand how difficult it is to find a family physician, and aim to make the process easier by asking interested patients to follow the following procedures prior to booking your first appointment:

• Please call our office to arrange a Meet & Greet appointment, this will usually occur in about 4-6 weeks (currently booking into January, 2020)

• Bring a list of all your medications and allergies. It would be also helpful if you have a simple summary of all your conditions. We may contact you prior to your first appointment with a New Patient Form to help streamline the information.

• Bring your previous medical files, or information on how to access them (e.g., address of your previous GP and their fax number.)

At the Meet & Greet appointment, your overall health will be reviewed & Clinic Policy will also be reviewed. If the physician and the clinic is a good fit, we then invite patients to sign off on the patient rostering form and book back for any active medical issues, including physicals
