Dear Patients,

We recognize that this past year and a half has been very difficult for everyone in different ways. All of us at Yonge Davisville Health Clinic have your best interest at heart. We ask that when speaking to us that you be patient and respectful. Our office will not tolerate any form of abuse towards our staff. If for some reason you are not happy, please let us know, calmly, and we will do our best to help you.

As Ontario continues to progress in reopening services that were closed due to the COVID-19 pandemic, many of you have been wondering how we are providing care to our patients.
We have worked very hard at meeting your needs over the past 20 months, while still following public health measures and restrictions. Our practices have steadily been moving to regular appointments in the office with a gradual re-opening of our waiting room. You have the option of scheduling a telephone/video appointment or in-office appointment. Please indicate your preference to Rosemary or Melanie and depending on your concern they will help book your appointment. As always, appointments are triaged based on urgency.

We ask you to be patient if you are booking a well visit, for although we have worked hard at staying on top of prevention tests (cancer screening, immunizations, BP and chronic disease monitoring), the pandemic has created a huge backlog. It will take many months to catch up. Priority booking for well visits is for those who are behind in these tests. Routine physical examinations for those who are up to date are being deferred at this time.

This year again we are offering the option of an extended health or Annual Fee Plan as an alternative to paying for each service not covered by OHIP. Examples of uninsured services include prescription renewals without an appointment, insurance and sick notes, missed appointments and medical supplies. Please carefully review the list of uninsured services, and which services are covered under the Annual Fee Plan, before deciding which option works best for you. There is no increase in fees for 2022.

Please note that uninsured services will not be provided until payment has been received. For prescription renewals, since the turn around can be up to 72 hours, we ask that you plan ahead to avoid being without medication. If your pharmacy sends an automatic refill notice, and you have not booked an appointment, you will be asked to pay before the renewal is approved.

As always, we invite patients who have financial difficulties to speak with us about a reduction/ exemption.

For your convenience, payment for uninsured services and the Annual Fee Plan can be made with a cheque made out to your doctor (not to the Clinic), over the telephone and on our website with Visa, Mastercard or debit card.

Please take a moment to notify us with any changes in your contact information and to visit our website to view our policies and stay updated with our clinic’s operations.

Best wishes and sincerely yours,
Dr Sidi Liu, Dr Shauna Phillips and Dr Sabrina Stables
